This online form is to be completed and submitted in the early phase of your event planning. The President's calendar fills up quickly, and while Dr. Darkwa would like to attend every event, it is not always possible for him to do so.

The earlier you submit this form, and the more detail you provide, the sooner we can respond to your request. You could also send (in hard copy) any additional information, brochures, or documents to further explain the event/organization.
